Support Coordination

mecwacare works with clients and their families to develop skills and increase their ability to live as independently as possible. Our Support Coordination team offers a range of in-home and community access options tailored to clients’ individual needs and choices.

Supports include informal, mainstream and community support, as well as clients’ NDIS-funded supports. It is time-limited and aims to support an NDIS participant to take part in the community. Support Coordination is a role funded by the NDIS.

  • There is a range of tasks available under mecwacare’s Support Coordination program:
  • Assist the participant to understand the supports in their plan
  • Assist the participant to navigate NDIS processes
  • Link participant to funded supports
  • Make referrals for any assessments in plans
  • Ensure equipment supports are managed
  • Ensure service bookings are complete
  • Explore mainstream and community supports
  • Assist the participant to resolve points of crisis
  • Ensure the participant is aware of complaints mechanisms

mecwacare can provide Level 1 and 2 Support Coordination for NDIS participants.

What do programs cost?

As a registered provider, mecwacare complies with the NDIS Price Guide. We encourage participants to regularly check the NDIS website here for the latest pricing information.

Can I use my NDIS funds for transport?

Participants now have greater flexibility in how they pay for transport – but only for certain types of trips. The updated NDIS Price Guide has created two new definitions of transport:

  1. Activity Based Transport: Transport “to, or from, or as part of, a community participation support”. This is transport delivered by a registered provider who will also be supporting the person at the destination.
  2. General Transport Services: Transport to “an activity that is not itself a support – or to a support that is delivered by another provider.” This category includes transport to places where the service is delivered by an unregistered provider, a provider other than the transport provider or a place where no support is being delivered. There is no increased flexibility for this type of trip, which still needs to be paid from a person’s transport budget.
