Clinical Team Leader South East Metro
Responsible To: General Manager Home Care and Nursing Services
Summary Of Position: This newly created leadership position based in our East Malvern head office is integral to the successful delivery of nursing and direct care services to clients of the Home Nursing and Care Services program. Part time 4 days per week.
The Clinical Leader South East Metro is responsible for:

You will be able to provide operational supervision and support to a team of Service Coordinators.

This position encompasses day to day management of client intake and distribution, immediate response to Service Coordinator and Direct Care Worker concerns and implementation of new processes and strategies identified through continuous improvement frameworks.
Competencies: The successful applicant will have:

Division 1 Nursing qualifications and registration;
- Strong clinical skills;
- Experience in frontline staff management;
- Excellent verbal and written; communication, with the ability to maintain and develop strong professional relationships;
- High level of interpersonal skills;
- Knowledge of and commitment to working within a continuous improvement framework;
- Ability to work appropriately with IT applications, including the Microsoft Office Suite; and
- Previous experience in Community or Aged Care is highly desirable.

Assessment Officer
Responsible To: Manager South East Region HNCS
Summary Of Position: Our Home Nursing and Care Services team are seeking a registered Division 1 Nurse or Allied Health Professional with experience in assessing clients in the home or a community setting. This role is located in Pakenham office.
The Assessment officer is responsible for;
- working with people who are elderly, people with disabilities, and their carers.
- understanding the care requirements for clients in the context of clinical, psychosocial and functional needs.
Competencies:
- Qualifications as a Division 1 Nurse or in an Allied Health Field;
- Recent experience in acute and/or community nursing;
- Sound clinical nursing skills;
- Strong communication skills, with a commitment to high quality customer service and ability to work with a range of stakeholders;
- Effective time management and problem solving skills; and
- A current drivers licence and registered vehicle.
Desirable:
- Experience in community based services and understanding of community funding streams.
- Knowledge of current practices and principles of comprehensive aged care assessments;
Knowledge of the Aged and Community Care industry would be an advantage.

Policy Advisor
Responsible To: Manager Quality and Risk
Summary Of Position:The Policy Advisor works with members of the Organisational Quality and Risk Department and service areas to enhance organisational processes, policies and procedures in line with regulatory and legislative requirements and best practice guidelines.
The Policy Advisor is responsible for:
- Coordinating the development, review and evaluation of policies, procedures and associated documentation that supports organisational systems and processes.
- The implementation, management and evaluation of quality documentation systems and processes, including document control.
- Interpreting legislative changes and identifying the impact for the organisation, including the need for policy review/development.
Competencies: The successful applicant will have:
- A relevant qualification and/or minimum of one years experience in policy development.
- Demonstrated experience in the evaluation of systems, processes, policies and procedures.
- Demonstrated experience in interpreting legislation, standards and codes of practice.
- Demonstrated knowledge and understanding of risk management, quality and continuous improvement principles and frameworks.
- Well developed organisational skills with the ability to prioritise, meet deadlines and attend to multiple tasks across a range of settings.
- Highly developed analytical skills
- Excellent written and verbal communication and interpersonal skills and
- Excellent administrative skills including Proficiency in Microsoft Software
