How to apply

Thank you for your interest in mecwacare Residential Aged Care Services.

If you wish to apply for a place in one of our facilities, please download and complete the following forms by clicking on the links:


Following completion of the forms, please choose one of the 3 options below to send the information to mecwacare:

Prospective residents will also need to gather the following documents for their application:

  • Current Aged Care Client Record (ACAS assessment)
  • Power of Attorney
  • Pension and Medicare Cards
  • A copy of the 'Statement of Residential Aged Care Fees and Assets Summary'. This statement is provided by the federal Department of Human Services (DHS). If you have not yet obtained this statement, follow the link below to the DHS website to obtain the request form:
